ELIZABETHTOWN — United Way of Bladen County will accept disaster assistance applications from Bladen County flood victims through Wednesday, Jan. 25, by 5 p.m.

Applications are available at 401 Mercer Mill Road in Elizabethtown.

Individuals must submit all requested forms and documentation below. Application that do not include the items below will be considered an incomplete application and will not be processed. All documents must be submitted at the same time.

— Picture ID verified by driver’s license, passport or other governmentissued ID.

— Proof of address where disaster occurred. If address on your driver’s license is not current, please provide a utility bill with disaster address visible (electric bill, water bill, etc.).

— Printed Copy of FEMA application(s).

— Printed Copy of ALL FEMA discussion letters (Note: if your documents have been misplaced or destroyed, instructions may be provided to you to retrieve documents from online. You will need to setup an online account which require username, email address and password.)

Note: Applications must be received by 5 p.m. on Wednesday, Jan. 25, at 401 Mercer Mill Road in Elizabethtown, N.C. 28337. Postmark date is not accepted. Applications must be complete including the above listed items.
